Combined Media Consent and Event Participant Release and Waiver of Liability

EVENT PARTICIPANT RELEASE AND WAIER OF LIABILITY

By participating in LightStyles + Builders Source Appliance Gallery's (Insert Name of Event) (the "Event"), hosted by one or more members ("Member Hosts") of LightStyles + Builders Source Appliance Gallery, I hereby release, discharge, and hold harmless LightStyles + Builders Source Appliance Gallery, its officers, directors, owners, employees, members, agents, and the Member Hosts from any and all liability arising directly or indirectly, now or in the future, from any injury, damage, loss, or expense related to my participation in the Event.

I acknowledge that LightStyles + Builders Source Appliance Gallery is not responsible for my conduct or any activities I choose to engage in during the Event.

I understand that alcohol may be available at the Event, and any decision to consume it is entirely my personal responsibility. Participation in consuming alcohol is optional, and no participant is required or expected to do so. I understand that LightStyles + Builders Source Appliance Gallery and the Member Hosts are not liable for any consequences resulting from my or another participant’s alcohol consumption. I recognize that alcohol is a drug and may have serious health and behavioral consequences.

I agree to indemnify, defend, and hold harmless LightStyles + Builders Source Appliance Gallery, its officers, directors, employees, owners, members, agents, and the Member Hosts from any claims, damages, costs, or expenses—including attorney fees—arising directly or indirectly from: (i) my alcohol consumption during the Event; (ii) my use of facilities provided by LightStyles + Builders Source Appliance Gallery or the Member Hosts; or (iii) any actions or incidents related to my participation in the Event.

This Release and Waiver of Liability is binding on my heirs, executors, administrators, successors, and assigns.


MEDIA CONSENT

I consent to LightStyles + Builders Source Appliance Gallery and its entities—Glenview Doors, Carriage House Doors, and INICIO Windows + Doors—capturing my name, voice, and/or image through photographs, videos, audio recordings, or other media ("Materials").

I authorize LightStyles + Builders Source Appliance Gallery and its entities to use, reproduce, publish, display, and distribute the Materials in educational or promotional materials, including but not limited to social media, websites, catalogs, articles, and brochures, without notifying me. I also permit them to identify me by name or initials.

I understand my participation is voluntary, and I will not receive compensation for signing this release or for the use of the Materials. All recordings and reproductions will remain the property of LightStyles + Builders Source Appliance Gallery and its entities.
